Services provided are intended to assist me in using natural means to support my health. I will not change or discontinue any medical treatment based upon my consultation with Hope and Holistic Healing LLC/ Amber - The Accessible Herbalist, and if I believe that modifications may be sensible in the light of these natural approaches, I agree to first discuss such changes with my prescribing medical physician and medical specialists. I agree to maintain regular visits with my primary care physician, medical specialists, and mental health professionals as appropriate. I agree to follow-up on referrals for medical care when necessary. If I believe that I have a condition that requires medical care, I will consult my primary care physician or an appropriate specialist.

Holistic and Natural Health practices are known as complementary, alternative, or holistic care, and may not be accepted by the larger community of medical professionals. Holistic means “whole person” and addresses a person’s mental/emotional/spiritual/physical/social/environmental needs. Natural Health consists of bodywork and energy work, lifestyle modification, mind/body techniques such as meditation, spiritual work, aromatherapy and botanicals, and modifying diet. Hope and Holistic Healing LLC/ Amber - The Accessible Herbalist is not a medical doctor, not a licensed medical or mental health professional, nor a licensed massage therapist. I understand that Amber - The Accessible Herbalist does not practice medicine. I understand that Hope and Holistic Healing LLC/ Amber - The Accessible Herbalist does not offer after hour services or provide any hospital-based services. Hope and Holistic Healing LLC/ Amber -The Accessible Herbalist is a holistic practitioner and wellness consultant; neither of which are licensed in Wisconsin. Amber is a Reiki Practitioner, certified life coach, herbalist, is trained in energy work, and has Associates Degree in Horticulture.
 

Services Provided
Bodywork/Energy work with Hope and Holistic Healing LLC/ Amber - The Accessible Herbalist includes Reiki, Chakra Work, and Herbal Medicine. Consultation/education consists of emotional and spiritual processing and discussion, education and discussion of nutritional issues, herbal remedies, aromatherapy, lifestyle modification, health, wellness, stress management, and mind/body techniques. Hope and Holistic Healing LLC/ Amber - The Accessible Herbalist will describe the nature of her consultation based on scientific literature, the expected prognosis without such care, and the anticipated costs, risks, benefits and experience of following various options.

Benefits
Modifying diet and lifestyle, natural remedies, and regular relaxation can improve immune function, decrease stress, and improve health and wellness. I understand that a core approach in natural health is achieving better health status through improvements in diet and lifestyle. The focus of natural care is to alleviate the underlying conditions that can bring about illness rather than the treatment of symptoms. While I may experience some immediate improvement from the use of natural remedies and botanical methods, I understand that the most effective results occur when I make a long-term commitment to rebuild my health.

Potential Risks and Healing Crisis
As with any method of care, natural health can involve some risk. When experiencing bodywork, energy work, and employing natural healing techniques such as herbs and nutrition, I understand that my body may detoxify (including dense energies and emotional patterns that no longer serve you a purpose) and I may experience aches, pains, or even new symptoms or the emergence of old symptoms as the body responds by shifting its balance and healing. This may be called a healing crisis. Through natural healing, the body’s self-healing mechanisms are awakened. As healing begins, symptoms reappear as part of the body's process of eliminating diseased cells and toxins from its tissues. This is a good sign and indicates deep levels of healing are occurring. Most clients report that they feel better than ever after the symptoms clear. I understand that I need to increase my water intake and amount of sleep following these services. While herbs, botanical products, vitamins, and supplements are generally available over-the-counter and are considered safe based upon their long history of use, many of them have not been widely tested. Negative reactions to natural remedies may include rare allergic reactions and very rarely, even shock or death. I understand that the interactions between herbs, and between herbs and drugs my medical physician might prescribe, are not yet well known, and that while unlikely, I could have an adverse reaction or experience a reduction or increase in the effect of other medications. This can have serious consequences for some medications, such as for the control of high blood pressure, insomnia, anxiety, or blood sugar. I understand that I should inform my physician about which herbs and botanical products I am considering taking before implementing. I understand that I should let my physician and pharmacist know what herbs I am taking at all times, particularly prior to surgery or other procedures.
 

Fees and Cancellation Policy
Hope and Holistic Healing LLC/ Amber -The Accessible Herbalist does not take third party insurance as payment. Therefore, I agree to pay Hope and Holistic Healing LLC in full at the prior to the beginning of each session and prior to purchasing any and all offers.

Cancellation/Rescheduling Policy
All sales, offers, and appointments are final and are NON REFUNDABLE under ANY circumstance.

No Guarantees
I waive all rights and agree to not hold Hope and Holistic Healing LLC legally responsible for any lack of progress or undesired results due to services received. With Natural Health and Healing, the ultimate responsibility is yours. In order to heal and improve health, YOU NEED to be willing to change and make modifications to your lifestyle.
